I consent for photographs and or video images to be taken of me by Club Beauty. I understand the images will be part of my medical record and may be used for purposes of medical teaching, training or for marketing purposes (website, print, digital or social media.)

By consenting to photographs and/or video images I understand I will not be compensated from Club Beauty.

This consent, as stated above, shall be a continuing consent for all procedures, past, present & future. A 90 day written notice must be received from the client asking to discontinue use.
